1. Letter of Authorization (LOA) for Local Number Port Request – The LOA gives us permission to request, on your behalf, transferring the existing number(s) to or from a different service provider.
2. Phone Bill with Your Name and Business Phone Number – Make sure to submit a copy of the phone bill to show that you pay for that number.
3. Do Not Cancel Your Phone Number – We will take of it for you. We need to port your number to a temporary phone number before we port it to the final destination. Please do not cancel your phone number before porting.
